This is now available! You can add up to 5 questions to your booking links, each one required or optional, plus a Company name field. Guests answer in text fields on the booking page, and their answers are added to the calendar event and to both confirmation emails.
Set them in a link’s More settings, or set defaults for new links in Settings → Share Availability. It’s part of version 2.83 on desktop and web. More in the Share your availability help article.
